Years ago, I used to watch Star Trek TNG, and the character DATA, would often interrupt the flow of a conversation to "query" a word, and confirm its meaning before allowing the conversation to continue. I always thought that he was onto something there...

For that to happen you need informed people who know what to do, or blind followers who do what they are told.
