They're trying to COERCE you into contract. Ask to keep the original copy, also, ask them the questions pete mentioned. As it's a contract - a bond, you have the right to have your stipulations upheld before you sign anything. Refusal of screening is just hospital policy bs. They're liable if anything goes wrong unless you sign a waiver saying if anything goes wrong it's your fault and thus you can't sue.
