I would add the person's cert # and ss#, and the regitration date of the event of the BC.

I have used "we" in the past as a way of inferring and reinforcing the fact that I'm not the PERSON and that there are multiple entities at play here. Maybe this isn't necessary and makes me sound like a loon? I will reevaluate my choice of words.
